I’m Kate, the Founder of Nexus.  I’ve been working in Colorado's Intellectual and Developmental Disabilities (I/DD) services since 2008. I began my career as a Direct Support Professional (DSP), providing support in a range of services including Supported Living Services (SLS), day services, employment services, respite care, and still hold the title of host home provider. Over the years, I transitioned into leadership roles, managing programs and guiding organizations toward success. My experience spans working with both small, one-person agencies and some of the largest I/DD service providers in Colorado, giving me a unique perspective on the industry.

As the founder of Nexus, I’m proud to lead an agency that acts as the link between individuals and families and the individualized care that they deserve. At Nexus, we provide access to a highly experienced multidisciplinary team, including experts in various supportive roles, all working together to ensure holistic, high-quality care for those we serve.

Serving Grand Junction and Colorado’s Western Slope is Tiffany Montaño.  Tiffany is a Licensed Clinical  Social Worker (LCSW) with 10+ years of experience supporting Colorado’s youth and IDD communities.  Tiffany has worked as a Case Manager, Residential Director, and Director of Behavioral Health before getting back to her true north, therapeutic services. Tiffany is currently providing Behavioral Counseling, Consultation, and Assessment services.

Kate Veeder, LCSW

Founder/Director

  • Kate has been deeply involved in Colorado's Intellectual and Developmental Disability (IDD) system since 2008. Over the years, she has had the privilege of managing a variety of programs, ranging from day services to employment initiatives, as well as respite and residential support and serve in many positions including Direct Support Professional, Program Director, Host Home Provider, Therapist, Clinical Director, Board President, and Executive Director of Organizational Strategy. Kate is a Licensed Clinical Social Worker (LCSW)whose work has always been driven by a passion for making a meaningful impact on the lives of individuals and families impacted by various disabilities, to help fill gaps in services, and to create innovative person-centered supports.

Tiffany Montaño, LCSW

Program Manager

  • With almost 15 years of experience supporting Colorado's youth and IDD communities, Tiffany has served in a variety of roles, including direct delivery of services, case management, and administration/leadership positions. Tiffany's success and vast experience makes her well-equipped to navigate complex human services systems and collaborate well with professionals of various roles. Tiffany is beyond grateful for the excellent clinical supervision and mentorship that she has received over the years, and feels compelled to give back to the social work and mental health professions in kind, and is delighted that she has found a way to make a positive impact on the world around her while still doing something she loves.

  • Farrah Haase brings over 12 years of dedicated experience in supporting individuals with intellectual and developmental disabilities (IDD). Her expertise spans direct care, job coaching, being a host home provider and program management. Farrah is passionate about creating opportunities that empower individuals to achieve independence, build confidence, and thrive in their communities.

    With a compassionate approach and a commitment to excellence, Farrah has a proven track record of fostering meaningful connections and delivering impactful services tailored to the unique needs of each individual. Her leadership and dedication continue to inspire positive change and promote inclusion in the IDD community.

Ashley Withington, RN

Registered Nurse

  • Ashley has supported individuals with disabilities for more than 20 years across various settings, including supported living services, residential and respite care, and para educator, among others. She holds an undergraduate degree in Human Development and Family Studies from Colorado State University and a Bachelor of Science in Nursing from Metropolitan State University of Denver. Her nursing expertise lies in acute rehabilitation, where the focus is on supporting individuals with acquired disabilities to regain independence. As a nursing consultant, Ashley offers person-centered nursing support to individuals with disabilities that address their medical, physical, emotional, and social needs, while also supporting and educating their caregivers to ensure a holistic and sustainable approach to care. She provides support with medication management including medication administration assessments, QMAP training, and Medication Administration Records.  Ashley provides multidisciplinary collaboration through the coordination of health care providers to create comprehensive care plans. With her expertise in direct patient care, as well as her extensive personal experience working with individuals with disabilities, she is uniquely equipped to offer patient-centered care to people with disabilities that integrate medical and psychosocial well-being.
